Employee Survey for Post Lockdown Views

Hi, we are considering sending a survey to all employees asking to share their concerns and things we should be considering coming out of lockdown (we already have a provisional plan but this only our thoughts so we want to hear what's on employee's minds). Has anyone done anything similar and if so, what questions did you ask? Did you use an external provider to support with this or did you just do it internally?
Thanks

Thank you very much for your suggestions, I'll take a look.

Thank you Adam, this is very helpful

Hey Tina, slightly different here as I've set it up as a wellbeing survey, but the fortnightly pulse survey on Survey Monkey (paid version) asks 8 questions and takes about 2 minutes to complete. 3 of these are free-text fields and this is really where we're getting the valuable insight. We are planning to transition this slightly, now that we've addressed immediate needs, to some more forward-looking questions, such as opinions on post-lockdown. We're also looking into pilot groups returning to the office to see what they need and what it feels like - within government guidance, of course.

I'm not aware of any intention for us to host a survey yet, but Mercer have some great online resources surroung the handling of the Coronavirus so might be worth checking that out. Willis Towers Watson also support employers in conducting all types of surveys.
Hope this helps
